I agree to wear a mask throughout my appointment as a basic Personal Protective Equipment (PPE) requirement.

I am aware of the existence of the risk of physical presence at the in-person appointment and my participation may cause injury or illness such as, but not limited to, influenza, MRSA, Covid-19, and I take full responsibility for my personal health and indemnify ISDA from any injury and/or damages arising from the appointment.

Within the last 14 days, I have not experienced symptoms that of fever, fatigue, difficulty breathing, or dry cough, or exhibiting any other symptoms relating to Covid-19 or any communicable disease.

Within the last 14 days, I have not, nor any member(s) of my household, traveled beyond 150mile from my home residence in any method of transportation.

Within the last 14 days, I have not, nor any member(s) of my household, been diagnosed to be infected by Covid-19.

I agree that I am fully and personally responsible for my own safety and actions during the in-person appointment, and I recognize that I may still be at risk of Covid-19 contraction. 

With full knowledge of risks involved, I hereby release, waive, discharge ISDA, its staff and affiliates from any and all liabilities, claims, demands, actions, and causes of action whatsoever, directly or indirectly arising out of or related to any loss, damage, injury, or death, that may be sustained by me as a result of the in-person appointment.

I agree to indemnify, defend, and hold harmless ISDA from and against any and all costs, expenses, damages, lawsuits, and/or liabilities or claims arising from the in-person appointment, either directly or indirectly.
